To participate in athletics, all athletes must have a physician complete PIAA Sections 5 & 6 once per school year. Exeter offers an opportunity to receive a low-cost athletic physical twice per school year. Please monitor ParentSquare to register for an appointment. Appointment announcements typically occur in early May for the upcoming school year, with another round of appointments available during the late fall.
Forms can also be completed by your own private physican, or at any urgent care facility.
Please note:
- Only one athletic physical is needed per school year.
- Physicals must occur on or after May 1 of the preceding school year.
- A physical conducted on May 1, 2026 for the 2026-27 school year is acceptable.
- A physical conducted on April 29, 2026 for the 2026-27 school year is not acceptable.

Registering for a Second or Third Sport
If your student-athlete has already registered for and participated in a sport during the current school year, you do not need to create a new SportsWare account or obtain another sports physical. -
- Go to SportsWare Online.
- Under SIGN IN, enter your existing login credentials.
- Click "Login" to access your athlete's account.
Proceed to Step 2
-
- Once logged in, navigate to your athlete's information.
- Select the upcoming sport under the appropriate field, for example:
- Sport 2: Winter sport
- Sport 3: Spring sport
- Save your changes.
Proceed to Step 3
-
Student-athletes participating in a second or third sport during the same school year must complete a PIAA Section 7 Recertification form.
- Open the appropriate PIAA Section 7 Recertification form for winter or spring in SportsWare.
- Complete the required information at the top of the form.
- Answer Questions 1-6, providing an explanation for any "Yes" responses.
- Add both the student-athlete's and parent/guardian's electronic signatures.
- Click "Save & Validate" and then "Continue to Signing" to complete the process.
Please note: You do not need to submit another PIAA Sections 5 & 6 physical form if your athlete already has a valid physical on file for the current school year. Depending on the answers provided on Section 7, additional medical clearance may be required.
Proceed to Step 4
-
If you have not already joined your new sport's ParentSquare/StudentSquare group to receive information from your coach, please follow the directions on this page to set up your account and find the link to join your team.
Notes
- This system does not generate/send notifications when your account toggles to “Cleared to Play.” Please check back in a few days to see if the status has toggled to “cleared.”
- Use the athlete’s legal name in all digital and physical forms; no nicknames, please.
- Complete fillable forms entirely before clicking “Save and Validate,” then “Continue to Signing.” The system will not allow you to reopen it once you’ve submitted a form.
